Advance Care Planning
If you were suddenly injured or became seriously ill, who would know your choices on health care?
Age, illness or accident may at some time make it hard for you to say what treatment you want and to understand your options.
Advance care planning means thinking now about what treatment and care you want in the future.
It can also mean:
- discussing this important issue with those closest to you and those you trust
- completing an Advance Health Directive or Enduring Power of Attorney.
